Mental Health Association of Nassau County (MHANC) is a not-for-profit membership organization dedicated to improving mental health in our community through advocacy, education, program development, and the delivery of direct services. Our organization is looking for a motivated individual with a calling to help adults with mental illness by becoming a Case Worker.
The Case Worker (1115 Waiver) provides comprehensive financial care coordination and benefits support to 1115 Waiver housing clients and Financial Management Representative Payee Program participants. This role helps clients secure and maintain public benefits necessary for housing stability and financial well-being, while also serving as an internal resource to MHANC staff to resolve benefits-related barriers to services.
The Case Worker (1115 Waiver) acts as a liaison between clients, MHANC program staff, and external benefit agencies—including SSA, Medicaid, Medicare, and DSS—to ensure accurate, timely documentation and uninterrupted access to entitlements
This position is funded through a grant through March 31, 2027. Employment is contingent upon the continued availability of grant funding. The position may be terminated if grant funding is reduced or discontinued before the end date. This does not prohibit the candidate from applying to other positions within the organization at that time.
Essential Duties & Responsibilities
Financial Care Coordination – 1115 Waiver & Representative Payee Clients
- Complete all trainings and implement the 1115 Waiver policies and procedures as it relates to enhanced housing services.
- Provide financial counseling to 1115 Waiver clients that are eligible for enhanced housing services through HEALI to determine specific financial housing needs.
- Liaison with Financial Coordinator and Community Health Worker to ensure 1115 Waiver clients have the proper documentation to receive housing related financial assistance.
Staff Support & Technical Assistance
- Serve as a benefits resource for MHANC staff, answering questions regarding eligibility, applications, documentation needs, and program rules.
- Assist staff and providers in completing benefit applications and troubleshooting issues that create barriers for clients.
- Provide guidance on how earnings and income changes affect public benefits.
- When necessary, assist staff in accompanying clients to SSA/DSS appointments and appeals.
Program Collaboration & Administrative Functions
- Participate in case conferencing and individual meetings with consumers as needed.
- Assist with departmental operations including phones, walk-ins, emails, mail distribution, and filing.
- Stay current on SSA, OTDA, Medicaid/Medicare, and other regulatory requirements; recommend policy updates.
- Perform additional duties as assigned.
Requirements
Education & Experience
- High school diploma required; Associate’s degree in Human Services, Social Work, Public Administration, or related field preferred.
- Experience with public benefits systems (SSA, DSS, Medicaid/Medicare, SNAP).
- Prior experience working with vulnerable populations (mental health, housing instability, financial insecurity) preferred.
- Strong communication, advocacy, and teaching skills to support staff and clients.
- Excellent documentation and time-management abilities.
- Proficiency in Microsoft Office; ability to work with databases and financial tracking systems.
- Commitment to confidentiality, equity, and trauma-informed service delivery.
Other Qualifications
- Valid NYS Driver’s License
- Microsoft Outlook Suite, Internet experience and working knowledge of computer operations including Excel, Word and Outlook.
- Demonstrated experience in effective professional communication, organization and multitasking.
- Must be able to handle simultaneous challenging situations in a high stress environment.
- Must have flexibility, personal integrity and the ability to work effectively with consumers, personnel and support agencies.
