IOC CASE MANAGER
Job Description
The Case Manager position requires working with severely, persistently mentally ill adults. You will provide comprehensive case management services to clients, facilitating client advocacy, and referral/service linkage via client outreach visits. You will teach clients the principles of Wellness and Recovery, self-care and symptom management and coach clients in the use of these skills in multiple environments and circumstances.
Qualifications
- Bachelor's Degree in a behavioral discipline: social work, psychology, sociology (required)
- Experience — Minimum of one year of experience working with severely persistently mentally ill individuals
- Knowledge — Understanding of mental illness, Wellness & Recovery Principles, Stages of Change, illness management, recovery and relapse prevention, Motivational Interviewing, and group process
- Flexibility — Ability to work flexible hours, which may include occasional weekends, evenings, and 24 hour on call availability
- Driver's License — A valid New Jersey Driver’s License in good standing with the Division of Motor Vehicles
Responsibilities
- Case management — Provide comprehensive case management services to adult clients, facilitating client advocacy, referral and service linkage via client outreach visits
- Education groups — Provide individual consultation and facilitation of Wellness & Recovery education groups
- Client consultation — Provide off site client consultation in the family home and/or other designated meeting site
- Service coordination — Provide linkage and service coordination to appropriate court ordered treatment modalities
- Resource knowledge — Develop extensive knowledge of mental health and social resources in the county, drawing upon team and collateral expertise as needed
- Team communication — Communicate to team regarding client interpersonal and environmental changes, barriers, and challenges for the development of effective treatment planning
- Client support — Facilitate groups and individual services to support clients' treatment goal progress and community integration
Experience
- Case management — 1 year (Preferred)
- Bachelor’s Degree in a behavioral discipline: social work, psychology , sociology
- Minimum of one year of experience working with severely persistently mentally ill individuals
