What are the responsibilities and job description for the Peer Screener Navigator position at Mental Health America Dutchess County?
Duties:
- Call provided list of Clients to screen and navigate for Health Related Social Needs.
- Enter data into UniteUs Platform and navigate clients to resources
- Use lived experience intentionaly to build authentic relationships.
- Follow department peer support service delivery protocols.
- Attend supervision with supervisor once a week.
- Have working knowledge of DC resources.
- Interest and ability to perform outreach with vunerable populations.
- Educate participants in multipule pathways of recovery paradigm,
- Assist participants with applying for benefits, entitlements, and other supports.
- Work with participants to identify strengths and provide non-clinical crisis support.
- Link participants to formal recovery supports, community resources, and support services.
- Identify and support linkages to community resources that support the peer participant’s goals and interests.
Qualifications:
- A valid NYS driver’s license and satisfactory driving record prefered.
· High School Diploma or GED
· FPA, YPA, NYSPS or CRPA registered through the NYCB or NYPSCB
· Competency in computers and ability to navigate electronic health record systems.
· Working knowledge of community resources, mental health, social services, and self-help programs.!
Pay: $23.50 per hour
