What are the responsibilities and job description for the Admissions and Social Services Director position at Menifee Meadows Nursing and Rehab?
Menifee Meadows Nursing and Rehab is currently looking for a Director of Social Services and Admissions. This individual will be responsible for the admission process and census development. Must be able to increase and maintain resident census by facilitating an efficient working relationship with the hospitals, physicians, physician groups, and appropriate case managers. Works with residents, staff members, and families on the front end of the admission process to ensure an efficient admission process.
Job Summary:
- Develops and utilizes existing tools and resources to grow and manage the relationship of each referral sources. Identifies program development opportunities to increase new admission and increase census.
- Execute pre-screening and admission paperwork at the facility within the local area for potential referrals, inquiries, and admission to identify their respective clinical need.
- Aggressively seek out potential residents and facilitate referral processing
- Acts as liaison between the facility and family members during the pre-admission and admission process. Educate residents, families, and visitors’ programs and services as needed.
- Handles tours, admission screening and initial inquiries
- Works with Director of Nursing to ensure all admissions are screened thoroughly and adequately prior to admission
- Ensures the admission packet is completed in a timely manner
- Facilitates the complete admission and discharge processes to ensure a quality customer experience
- Complete post-admission and discharge satisfaction surveys with residents and families.
- Lead the development and implementation of social service programs that meet the needs of individuals within the facility
- Manage program budgets, ensuring financial accountability and sustainability.
- Develop and maintain relationships with community organizations, stakeholders, and government agencies to promote collaboration and resource sharing.
- Oversee administrative functions related to program management, including reporting, documentation, and compliance with state and federal regulations.
- Advocate for the needs of clients within the organization and in the broader community.
- Strong background in social work with experience in behavioral health.
- Excellent administrative skills to manage multiple projects effectively.
- Proven experience in program development tailored to individuals with developmental disabilities, dementia, and other psychosocial needs.
- Education and Skills:
- Two years working as an Admissions Ambassador/Coordinator
- Medical knowledge and terminology
- Insurance authorization experience preferred
- Understands healthcare reimbursement programs
- Must be energetic and enthusiastic, able to make connections within the community
- Must have good oral, written, and organization skills to communicate effectively with residents, family members, visitors, government agencies, staff members, and general public
