Care Coordinator
We are seeking a highly skilled Care Coordinator to join our team at Mendocino Coast Clinics.
About the Role:
The Care Coordinator will work closely with patients, families, and healthcare professionals to ensure that our patients receive high-quality care. They will be responsible for coordinating patient care plans, scheduling appointments, and implementing treatment plans.
Key Responsibilities:
- Patient Assessment and Planning:
Assess new patients by gathering information, reviewing diagnoses, and performing a part of the Welcome to Medicare meeting for new patients. - Care Plan Development and Implementation:
Work with providers to develop and implement care plans, revising them as needed based on patient progress and provider input. - Scheduling and Coordination:
Work with patients, providers, and other medical professionals to schedule appointments and implement treatment plans. - Patient Education:
Educate patients and their families about treatment options. - Additional Services:
Provide symptom management and emotional support to patients.
Other Duties:
- Electronic Medical Records (EMR):
Enter, edit, and update patient demographic and clinical information accurately to ensure quality data. - Patient Confidentiality:
Maintain strict patient confidentiality. - Patient Files:
Update immunization records, health maintenance flow sheets, and enter all procedures performed, including blood draws, vital signs, wound irrigations, injections, etc. - Documentation:
Note all phone conversations in writing and sign.
Requirements:
- Nursing Degree:
Graduation from an accredited program of Nursing, either LVN or RN. - Clinic Experience:
At least three (3) years Clinic Experience in out-patient care. - Licensure:
Possession of a valid certificate of registration as a Licensed Nurse. - CPR Certification:
Valid certificate of training in Cardio-Pulmonary Resuscitation.
