Position Overview:We are seeking a dedicated and experienced Wound Care Nurse Practitioner to join our team in Southern Colorado. This role involves providing mobile wound care services, averaging 6-15 patient visits per day, 4-5 days per week. The ideal candidate will have a strong background in wound care management and be skilled in debridement and the placement of skin substitute grafts.
Key Responsibilities:
Patient Care:
- Conduct comprehensive assessments of patients' wound care needs.
- Perform wound debridement and perform procedures as indicated.
- Develop and implement individualized wound care treatment plans.
- Monitor and document patient progress and adjust treatments as necessary.
Coordination and Communication:
- Collaborate with long-term care facilities, assisted living facilities, family members, and home health care companies to ensure coordinated care.
- Communicate effectively with patients, families, and other healthcare providers to educate and inform about treatment plans and progress.
- Maintain accurate and timely documentation of patient visits and care provided.
Compliance and Quality Improvement:
- Ensure compliance with all regulatory requirements and company policies.
- Participate in quality improvement initiatives to enhance patient care and service delivery.
Qualifications:
Education and Certification:
- Master’s or Doctorate degree in Nursing.
- Current Nurse Practitioner license in the state of practice.
- Certification in Wound Care (WOCN, CWCN, CWS or similar) is preferred.
Experience:
- Minimum of 2 years of experience as a nurse practitioner
- Experience with wound care and mobile healthcare delivery is a plus.
Skills:
- Proficient in advanced wound care.
- Strong organizational and time management skills.
- Excellent interpersonal and communication skills.
- Ability to work independently and make clinical decisions autonomously.
Compensation:
The salary range is $105k - $145k. This is a mobile position, you must have reliable means of transportation and mileage reimbursement is provided.
Work Schedule:
- 4-5 days per week
- Average of 6-15 patient visits per day
