Wage Range:
Master’s Level Trained LSW: $60,000/year-$70,000/year
LCSW or LCPC: $70,000/year – $90,000/year
Required Qualifications
- Proven work experience as a Behavior Health Provider
- Valid IL Driver’s License
- Working knowledge of social theories and practices
- Social perceptiveness and empathy
- Ability to build and maintain professional helping relationships
- Ability to relate and communicate with diverse population and groups
- Resilience along with ability to assess situations
- Ability to practice in State of IL with Licensed Social Worker (LSW) credentials or as a licensed mental health practitioner.
Preferred Qualifications:
1. Ability to practice in the State of Illinois at the Independent level [Licensed Clinical Social Worker (LCSW), Licensed Clinical Professional Counselor (LCPC)]
Note: Gavin O’Neil will move to telehealth early in 2026. Additionally, he will still be involved with the existing mobile clinic, as well as the forthcoming second unit.
Job Summary:
With a primary focus on the Mobile Unit that visits area schools, this position is responsible for providing initial diagnostic evaluations and continued therapy as part of the behavioral health team. This position will be housed in the Mobile Unit (school age) during the school year and at the Memorial Medical Building when not in the Mobile Unit. This position requires providing services for school age children (outside of the Mobile Unit, support throughout the lifespan preferred, but negotiable) in a manner that demonstrates an understanding of the functional/developmental age of the individual served. This position requires full understanding and active participation in fulfilling the Mission of Memorial Hospital.
Primary Job Responsibilities:
- Initial mental health evaluations resulting in individualized treatment plans
- Plan, coordinate, manage and implement support packages to help clients deal with difficulties and overcome dependencies
- Interview service users and assess their current condition, needs, strengths and weaknesses
- Assessing clients using standardized assessment/screening tools, measures, etc.
- Address each case as a unit and set tailored measurable goals
- Monitor and evaluate clients’ progress and modify treatment plans accordingly
- Individual psychotherapy with children, adults and families
- Offer information and counseling on the best course of action during sessions
- Provide group psychoeducation/treatment related to chronic medical conditions
- Maintain accurate records and report on clients’ status
- Attend case conferences and provide family/client support in school (IEP/504 meetings)
- Act as a keyworker and cooperate with multidisciplinary teams
- Provide information and referral services.
- Refer clients to community services to help them in recovery
- Crisis assessments/triage with appropriate interventions
- Provision of other services related to wellness
- Documentation of services provided in accordance with requirements
- Other related activities designed to provide integrated health care services
- Documentation and scheduling in electronic health record
- May be required to drive mobile unit
- Other duties as assigned
