What are the responsibilities and job description for the Population Health Scheduler - Accountable Care Organization - Days - PRN position at Memorial Hospital at Gulfport?
Qualifications
- Coordinates and Schedules Annual Wellness/Preventive appointments
- Schedules appointments for Population Health Navigators and Primary Care Providers
- Verifies insurance benefits, frequency, and eligibility for wellness appointments
- Provides education specific to population health measures and wellness appointments.
- Collaborates with interdisciplinary staff to coordinate and schedule for Providers and navigators
- Cancels and reschedules patients as requested by patient, provider, navigator, or staff
- Consistently reviews all Population Health Navigator schedules to ensure appropriate number of patients scheduled per day
- Monitors multiple message pools to ensure continuity of care for patient population
- Maintains electronic medical records by entering accurate patient demographic, insurance, and scheduling information
- Enters and verifies all required demographics are current and up to date to ensure accurate electronic health record
- Verifies patient billing information and correct contact information
- Communicates with all clinics using the messaging system in the electronic health record
- Schedules on demand virtual telehealth appointments
- Maintains the accuracy of the on-demand provider schedule
- Secures appropriate payments for on demand telehealth appointments
- Communicates with on-call provider to ensure a warm transfer for a seamless transition
- Participates as a member of the team to achieve business goals, quality outcomes, and customer satisfaction
- Assists with providing access to primary care providers
- Answers general patient questions related to all operations of the ambulatory clinics
- Maintains up to date information on clinic hours of operations and updates patients of any scheduling concerns
- Communicates change in clinic hours when necessary
- Regular and timely attendance is required for this job
Education: High School or GED equivalent.
