Compensation: Based on Experience
Education:
High school diploma or equivalent required.
- Associate or bachelor’s degree in public health, social work, human services, or a related field is preferred.
Ongoing training in community health practices and health education is advantageous.
Regulatory Requirements:
Valid driver’s license and proof of insurance, if the role involves travel.
- Background check and health screenings may be required depending on employer policies and funding sources.
CPR/First Aid certification may be required or preferred.
Responsibilities:
Conduct outreach and education in homes, clinics, community centers, and other local settings.
- Assist individuals and families in navigating healthcare and social service systems.
- Provide culturally appropriate health education and information.
- Help clients access medical, behavioral health, housing, food, and other essential services.
- Advocate on behalf of individuals and communities to reduce barriers to care.
- Build trusting relationships with clients to support health behavior change.
- Coordinate with healthcare providers and social service agencies for follow-up and care management.
Document client interactions and maintain accurate records in compliance with privacy standards.
Attend and participate in community events, training, and staff meetings.
