Position Overview
The Clinical Liaison RN is a key representative of Melodia Care Inc., responsible for building and maintaining strong clinical and professional relationships with large hospitals and major healthcare institutions. This role ensures an efficient, accurate, and seamless transition of eligible patients from hospital settings to Melodia Care’s hospice and home health services.
The Clinical Liaison RN acts as the bridge between hospital systems, Melodia Care’s head office, and patients/families by conducting clinical assessments, coordinating referrals, educating care teams, and supporting timely discharge planning.
This is a high-visibility role requiring excellent clinical judgment, professionalism, and the ability to work autonomously within fast-paced hospital environments.
Key Responsibilities
1. Hospital Relationship Management
- Serve as the primary on-site representative of Melodia Care Inc. within assigned hospitals and major healthcare facilities.
- Build, maintain, and strengthen relationships with hospital leadership, physicians, case managers, discharge planners, and nursing units.
- Promote Melodia Care's hospice and home health capabilities through professional communication, education, and collaboration.
- Attend clinical coordination meetings, interdisciplinary rounds, and departmental huddles as required.
2. Patient Referral Assessment & Coordination
- Review patient referrals for hospice or home health eligibility based on clinical indicators, medical records, functional status, and psychosocial needs.
- Conduct bedside patient assessments as needed to determine clinical appropriateness for Melodia Care services.
- Communicate and collaborate with attending physicians, hospitalists, specialists, and nurse practitioners regarding patient status and care needs.
- Provide timely and accurate referral updates to Melodia Care’s head office, intake department, and clinical management team.
3. Discharge Planning & Transition Support
- Facilitate smooth transitions from hospital to home, SNF, assisted living, or other approved care settings.
- Coordinate with Melodia Care’s internal teams to ensure all required orders, consents, insurance verifications, and documentation are completed.
- Ensure durable medical equipment (DME), medications, and home services are arranged prior to discharge.
- Serve as a clinical resource to families, addressing concerns, explaining care plans, and ensuring clear understanding of provided services.
4. Patient & Family Education
- Provide compassionate, thorough education to patients and families regarding:
- Hospice philosophy and eligibility
- Home health services
- Symptom management expectations
- Transition process and care coordination
- Assist with completing necessary documents such as hospice consents, election statements, and service agreements.
5. Internal Collaboration with Melodia Care
- Collaborate closely with the Head Office, Clinical Leadership, Intake Team, and Interdisciplinary Group (IDG) to streamline admissions.
- Document all activities accurately in the EMR system and maintain referral metrics.
- Participate in internal case reviews, liaison updates, and operational planning meetings.
6. Compliance & Professional Standards
- Ensure all communication, documentation, and patient interactions comply with federal, state, and company regulatory standards.
- Maintain confidentiality and adhere to HIPAA guidelines at all times.
- Represent Melodia Care with the highest level of professionalism, clinical integrity, and ethical conduct.
Qualifications & RequirementsEducation
- Required: Active and unencumbered Registered Nurse (RN) license in California.
- Preferred: Bachelor of Science in Nursing (BSN) or equivalent.
- Valid Driver’s license
Experience
- Required:
- Minimum 1 year of direct clinical nursing experience (acute care, hospital, home health, hospice, or similar).
Preferred:
- Experience in liaison work, discharge planning, case management, or referral coordination.
- Experience working within large hospital systems.
Skills & Competencies

Physical & Work Environment Requirements
- Ability to walk hospital units, meet patients at bedside, and attend meetings across campus.
- Exposure to typical hospital conditions including infectious diseases, body fluids, and clinical settings.
- Flexibility to travel between hospital sites and Melodia Care’s head office if required.
