JOB SUMMARY: Plans, develops, organizes, implements, evaluates, and directs activity programs in accordance with current federal, state and local standards as well as the established policies and procedures of MMHS, to assure that the spiritual development, emotional, recreational, and social needs of the residents are met and maintained on an individual basis.
When appropriately called upon, will performs patient care activities and related non-professional services necessary in caring for the personal needs and comfort of residence/patients; assists in maintenance of safe and clean environment. He/she will individualize residence/patient care based upon the age appropriate and developmental needs. Should be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the residence/patients served on his/her unit.
PERFORMANCE DIMENSIONS AND TASKS
Essential Function
Activities Coordinator
1. Develops, implements and supervises an active program for patients in SNF.
a. Develops an activity program based on the assessed needs of the individual resident seen in the SNF.
b. Performs an assessment of individual residents to determine activity needs upon admission charts information on appropriate forms within five days.
c. Implements the program developed and supervises those persons assisting the activities program.
d. Regularly involves the patient/family in planning activities when possible.
2. Conducts inservice programs as required.
a. Plans and conducts inservice programs for the SNF staff based on assessed needs.
b. Keeps appropriate records of all training programs.
3. Coordinates the activity schedule.
a. Prepares a monthly activity calendar and schedules programs in such a manner as to avoid conflict with other facility functions.
b. Maintains a current list of residents who are physically unable to participate in scheduled activities.
c. Prepares alternate programs for residents who are physically unable to leave their rooms.
d. Posts activities schedule conspicuously in large print for the information of residents and staff.
4. Records resident’s activity plan on clinical record.
a. Maintains progress notes specific to the residents’ clinical record at least quarterly, more frequently if needed.
b. Maintains a current record for each resident reflecting the type and frequency of activities provided
c. Records names of all residents participating in each activity.
5. Requests and maintains equipment and supplies necessary to conduct the activity program.
a. Is cost-conscious when ordering and using equipment and supplies.
b. Maintains appropriate record of use of supplies and equipment in order to plan effectively for future programs.
c. Requests supplies and equipment based on needs of program and available monies.
d. Receives, places and transfers calls using appropriate telephone etiquette. Handles telephone information requests with courtesy, accuracy and respect for confidentiality.
CNA
1. Assists in preparation of SNF unit, residence/patients’ (i.e. patient) rooms and beds.
a. Prepares SNF unit to receive patients using established department procedure.
b. Checks patient room to ensure that all supplies and equipment required for admission are present.
2. Assists in and may accompany patient in the admission, transfer and discharge procedure.
a. Assists patients in the admission, transfer and discharge procedure as assigned following established department procedure.
b. Uses proper techniques of body mechanics to protect patient and self.
c. Provides for patient’s safety by using appropriate preventative safety measures, i.e., side rails, restraints, patient education.
d. Performs and documents patient admission and discharge data correctly and accurately as assigned.
3. Provides direct personal care, general nursing care and routine treatment for a group of patients.
a. Demonstrates competence in providing morning, evening and general care according to established department procedures.
b. Demonstrates awareness of patient’s comfort and safety when performing any personal care activities.
c. Demonstrates competence in performing treatment procedures such as enemas, application of heat/cold, sitz baths and skin care.
d. Demonstrates awareness of patient safety, comfort and privacy when performing any treatment.
e. Records/reports to RN on duty any treatment results, observations, patient complaints, variations from normal and other pertinent information regarding the condition of patients.
f. Demonstrates ability to perform general nursing care for patients following established procedures, i.e. positioning patients, lifting and turning patients, applying and utilizing special equipment, assisting in use of bedpan, urinal or commode.
g. Takes and records temperature, pulse, respiration, blood pressure, height, weight, intake/output and other measurements as applicable in an accurate and precise manner.
h. Records pertinent patient care activity information on appropriate flow sheets in an accurate/timely manner; signs name to these records.
i. Distributes water and other nourishments according to patient needs.
j. Demonstrates competence in performing various patient care activities for the patient in isolation according to Infection Control policies. Uses Standard Precautions.
k. Demonstrates competence in prescribed hand-washing and aseptic techniques.
l. Answers any patient call light. Personally assists patient with requests or reports request to appropriate personnel.
m. Anticipates needs of patients and checks on assigned patients in a consistent manner (at least once and hour)
n. Demonstrates a cooperative and positive attitude when assisting patients.
o. Ensures that patient call light is within patient’s reach and available for use.
p. Assists with the disposition of patient’s clothing and personnel property, i.e., prosthetic
q. Devices, glasses, etc., secures items in accordance with established department policy.
r. Records all required information on appropriate form and provides for safe care of patients valuables.
4. Assists licensed staff in complicated treatment procedures.
a. Assists licensed staff in performing complicated treatment procedures as requested that are within scope of responsibility.
b. Demonstrates ability to follow directions and to perform specific duties under the supervision of licensed staff.
c. May participate in supportive activities in a code procedure as requested.
5. May be directed to provide support to residents/patients while engaged in social activities or appointments where applicable.
a. Must maintain a current Driver’s License and automobile insurance.
b. Can be directed to utilize company vehicles to (act as a driver) transport residents/patients to field trips or appointments.
c. May be directed to be present and support residents/patients while they are engaged in socials, field trips, or appointments as needed.
QUALIFICATIONS
- Minimum of (2) two years’ experience in a social or recreational program within the past five years, one of which was full-time in a patient activities program in a healthcare setting, or
- Be an occupational therapist, art therapist, music therapist, dance therapist, recreation therapist or occupational therapy assistant, or
- Have satisfactorily completed at least 36 hours of training in a course designed specifically for this position within one year of hire or transfer to position.
- Current Certified Nurse Assistant certificate – Required
- Current AHA BLS certificate – Required
- Must sign Mee Memorial Hospital Drivers Agreement
- Provide Valid California Driver License – upon hire and prior to expiration
- Provide Current DMV Record – upon hire and annually during anniversary month
- Provide Proof of California mandatory limits of liability motor vehicle insurance – upon hire
