What are the responsibilities and job description for the Social Services part time position at Medway Country Manor?
Medway Country Manor is seeking an experienced part time Social Worker Needed for 24 hours a week with a flexible schedule. Medway Country Manor is a 120-bed privately owned skilled nursing facility. The ideal candidate will posess:
* Minimum of a Bachelors of Arts in Social Work
* Current Massachusetts Social Work license, LSW minimum
* Three years experience in a skilled nursing home, working with a diverse population, long term care, dementia, behaviors
* Exceptional interview skills with residents and family members
* Excellent phone and communication skills.
* Knowledge of community resources
* Must be able to speak, read, and write in English
* Ability to compile social history data, participate in assessments and develop care plans
* Assist residents with their adjustment to the facility and their changing health conditions
* Participate as part of a team and act in the best interests of the resident. Attending care plan meetings, family meetings, coordinating with hospice
Job Type: Part-time
Pay: $40.00 - $45.00 per hour
