Position Summary
The Hospice RN Case Manager is responsible for coordinating and delivering high-quality hospice nursing care to patients in their homes or facilities. You will oversee the plan of care, collaborate with the interdisciplinary team, and support families through education and compassionate clinical guidance.

Responsibilities
- Manage hospice patient caseload (home or facility-based).
- Develop, implement, and update individualized care plans.
- Perform patient assessments, monitor symptoms, and maintain clinical documentation.
- Coordinate closely with physicians, social workers, chaplains, and other IDT members.
- Provide education and emotional support to families and caregivers.
- Participate in on-call rotation as needed.
- Ensure compliance with hospice standards and regulations.
Qualifications
- Valid CA Registered Nurse (RN) license.
- Minimum 1 year hospice or home health experience preferred.
- Strong clinical assessment and communication skills.
- Reliable transportation and valid driver’s license.
- Compassionate and comfortable working with end-of-life care.
- Must be willing to travel within assigned service areas, including San Francisco, San Mateo, and San Jose.
