Responsibilities

• Conduct comprehensive patient evaluations and physical examinations
• Diagnose and manage acute and chronic medical conditions
• Order and interpret diagnostic tests and laboratory studies
• Prescribe medications and treatment plans within scope of practice
• Provide preventive care, wellness counseling, and chronic disease management
• Educate patients regarding medications, treatment plans, and healthy lifestyle choices
• Collaborate with physicians, behavioral health professionals, nursing staff, and care coordinators to deliver exceptional patient care

Qualifications

• Licensed Family Nurse Practitioner (FNP) or Physician Assistant (PA)
• Minimum of 2 years of primary care experience preferred
• Strong clinical and communication skills
• Commitment to serving diverse and underserved patient populations
• Indiana license (or ability to obtain)
