This is a full-time outpatient position focused on continuity of care, preventative medicine, chronic disease management, and relationship-driven patient care. Providers typically see 16–22 patients per day in a well-supported clinic setting with experienced nursing and support staff.

Requirements:
- Licensed Nurse Practitioner or Physician Assistant
- Prior outpatient primary care experience preferred
- Comfortable practicing independently within a team-based environment
- New York license preferred or ability to obtain
Compensation & Benefits:
- $120,000 – $160,000 depending on experience
