A growing behavioral health and addiction treatment organization in Northwest Ohio is seeking a Case Manager II to join its Women's Residential Treatment Program. This position offers the opportunity to work directly with women in recovery, helping them build stability, access resources, and achieve long-term success in a supportive residential environment.
The Case Manager serves as a key member of the treatment team, coordinating services and supporting clients throughout their recovery journey. Responsibilities include connecting clients with medical, legal, housing, employment, and community resources; assisting with discharge planning; monitoring treatment progress; conducting ongoing needs assessments; supporting crisis intervention efforts; and maintaining accurate documentation. The role also involves collaborating closely with counselors, nurses, medical providers, and community partners to ensure coordinated, client-centered care.
The schedule consists primarily of first-shift hours with one to two evenings per week. Occasional weekend coverage may be needed based on program needs. Due to the nature of the residential program, female candidates are preferred.
Qualifications
- Bachelor's degree required
- LSW preferred; LCDC-II, CDCA, or similar behavioral health credential considered
- Experience in mental health, substance use treatment, case management, or residential services preferred
- Valid driver's license and ability to transport clients when necessary
- Strong communication, organization, advocacy, and community resource navigation skills
