Bilingual (Spanish‑Speaking) LPN – In‑Home Primary Care Visits

About the Position

We are hiring an experienced, reliable LPN to join our team. This role is fully community-based, providing in-home visits while working closely with our telehealth providers. You’ll be the clinical presence inside patients’ homes—handling assessments, basic treatments, and bloodwork while ensuring each patient receives quality, compassionate care. Our practice is seeking a compassionate and skilled LPN to provide in-home primary care visits. In this role, you will work closely with our telehealth providers, serving as the hands-on clinical support for patients in their homes. This is a great opportunity to deliver personalized care, build patient relationships, and work in a flexible, community-based environment.

What You’ll Be Doing

• Perform in-home visits for common primary care needs, including general illness, earaches, virus testing, rashes, allergies, eye infections, sore throats, and sunburn care.

• Conduct phlebotomy and obtain bloodwork as needed during visits.

• Collaborate with telehealth providers to ensure coordinated, comprehensive patient care.

• Document all visits thoroughly and communicate any follow-up needs to the clinical team.

What We’re Looking For

• Valid LPN license in New York State.

• Phlebotomy experience required.

• Prior in-home care, primary care, or similar clinical experience preferred.

• Strong communication skills and a compassionate, patient-focused approach.

• Ability to travel locally within the Hempstead area.

• Must be Spanish speaking.
