We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team and provide high-quality care in a skilled nursing facility setting. Key Responsibilities:
- Serve as the primary care provider for both long- and short-term residents.
- Conduct comprehensive health assessments, physical examinations, and review of medical histories.
- Collaborate with attending physicians, nurses, and facility staff to deliver coordinated, evidence-based care.
- Build trusted relationships with residents and their families; educate and develop individualized care plans.
- Ensure compliance with all federal, state, and local healthcare regulations and standards.
- Maintain professional development through continuing education and participation in professional organizations.
- Support a clean, safe, and respectful care environment aligned with organizational values.
- Ensure accurate, timely, and thorough documentation of all patient interactions and care provided.
Qualifications:
- Must be a Board-Certified Nurse Practitioner (NP).
- Strong clinical skills and attention to detail.
- Excellent communication and teamwork abilities.
- Demonstrated commitment to patient-centered care.
- Experience in a hospital or long-term care setting preferred (training will be provided).
- Competencies in health promotion, infection control, medication administration, and pain management.

Pay: $135,000.00 - $160,000.00 per year
