'Key Responsibilities

• Provide comprehensive medical care toSNFpatients, including assessments, diagnosis, and treatment planning
• Responsible forfull medical management and primary care
• Ensureaccurateandtimelydocumentation in EMR

Required Qualifications

• Active Physician Assistantlicense in Illinois
• Active DEA license (or ability to obtain prior to start)
• Minimum 1 year ofinternal medicineexperience as aPA
• Experience managing adult/geriatric patient populations
• Strong clinical decision-making and time management skills
• Comfortable working independently in an SNF setting

• Skilled Nursing Facility (SNF) or post-acute care experience
• Background in internal medicine,geriatricsor inpatient hospitalist

Physical Requirements

• Prolonged periods of standing, walking, and sitting while working with patients as well as working on a computer.
• Must be able tolift upto 50 pounds.
• May be exposed to bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.
• Must wear the required PPE per company and/or facility requirements.
