Registered Nurse (RN) – Home Health
Responsibilities:
- Provide and coordinate patient care using the nursing process and professional nursing standards.
- Conduct initial and ongoing assessments of patients’ physical, psychosocial, and environmental needs.
- Develop, implement, and update individualized plans of care.
- Coordinate discharge planning with patients, physicians, caregivers, and other healthcare professionals.
- Complete OASIS assessments and required documentation.
- Supervise CNAs and coordinate care with other healthcare workers.
- Educate patients, families, and caregivers on healthcare needs and treatment plans.
- Follow federal, state, and agency regulations.
- Maintain a clean, safe environment and protect patient privacy and confidentiality.
Requirements:
- Home Health experience — Required.
- Active BLS certification or willingness to obtain — Required.
- Valid driver’s license and car insurance — Required.
- Active Illinois RN license; copy must be included with submission — Required.
- Strong Med-Surg experience with some Home Health experience may be considered.
- OASIS and Start of Care experience — Strongly Preferred.
- COVID-19 and flu vaccinations required; declinations will not be accepted.
- Local candidates preferred; candidates must be within reasonable distance of the assigned territory.
Salary : $55 - $60
