Optional weekend shifts at $70/hr.Salary Range: $83,075.20 - $128,773.00Setting: HospitalDress Code: ScrubsStart Date: ASAPNot a government positionIT equipment providedSUMMARYThe RN Case Manager collaborates with patients, families, physicians, social workers, and the interdisciplinary team to coordinate patient-centered care across the continuum. This role focuses on safe, effective transitions of care, eliminating barriers to clinical services, and supporting optimal clinical, quality, and cost outcomes from pre-admission through post-discharge.ESSENTIAL FUNCTIONSDevelops safe, sustainable transition plans tailored to patient needs and resources.Screens all assigned patients to identify readmission risks, strengths, and clinical needs.Initiates and documents transition planning for assigned caseload.Collaborates with the healthcare team to ensure care plans are developed, implemented, and adjusted as needed.Coordinates delivery of clinical services and works with physicians, specialists, and payors on care strategies.Serves as a consultant on managed care, discharge planning, reimbursement, and levels of care.Leads and coordinates multidisciplinary rounds.Identifies high-risk behaviors and social determinants impacting outcomes and readmission risk.Communicates with community-based case managers to ensure continuity of care.Maintains documentation in EMR and care management systems.Participates in process improvement, data collection, and committee meetings.Educates patients and families to promote independence and improved outcomes.Mobilizes resources and responds to variances in care plans.REQUIRED QUALIFICATIONS BSN and completion of accredited RN program.Active Maryland RN license.Minimum 3-5 years nursing experience or 5 years case management experience.Psych experience required.Case management experience preferred.For California Applicants:We will consider for employment all qualified Applicants, including those with criminal histories, in a manner consistent with the requirements of applicable federal, state and local laws, including the City of Los Angeles' Fair Chance Initiative for Hiring Ordinance (FCIHO), Los Angeles Fair Chance Ordinance for Employers (ULAC), The San Francisco Fair Chance Ordinance (FCO) , and the California Fair Chance Act (CFCA).This position is subject to a background check based on its job duties, which may include patient care, working with vulnerable populations, access to financial and confidential information, driving, working with heavy machinery, or working in a warehouse or laboratory environment.
