Overview
The Nurse Practitioner (Certified Wound Care Nurse) will deliver comprehensive wound management and skin disorder care to adult and geriatric patients in their homes, assisted living communities, and skilled nursing facilities. Care is primarily in-person, with virtual support as appropriate.
This role is part of a value-based wound care organization focused on making wound care more accessible, affordable, and evidence-driven. The mission is to provide medically necessary, high-quality, and individualized care through collaborative partnerships with patients and providers.
Unlike traditional mobile wound clinics, this model emphasizes early engagement, clinical integrity, and close coordination with the healthcare team to achieve optimal patient outcomes.
Key Responsibilities
Provide comprehensive assessment, evaluation, and management of patients with acute and chronic wounds and related skin conditions.
Perform wound debridement and other medically necessary procedures in accordance with CMS guidelines.
Develop individualized treatment plans, coordinate care with interdisciplinary teams, and document all encounters accurately.
Utilize both in-person and virtual visits to ensure continuity of care.
Maintain compliance with clinical, regulatory, and documentation standards.
Collaborate with clinical leadership to continuously improve care delivery.
Qualifications
Education & Licensure
Master's Degree in Nursing required.
Licensed Registered Nurse (RN) and Board-Certified Nurse Practitioner (NP) in the state of practice.
Valid driver's license and reliable transportation.
Certification (Required or Obtainable within 90 Days):
Current Wound Care Certification required (may be obtained within 90 days of hire).
Preferred Certifications:
CWCN (Certified Wound Care Nurse) through WOCNCB.
WOCN Society accredited educational program completion.
Alternative certifications: Wound Care Certified (WCC) or Certified Wound Specialist (CWS) with at least 3 years of wound care experience.
Experience:
Minimum 3 years of full-time experience in acute care, skilled nursing, home health, or outpatient wound care.
Minimum 1 year of NP experience in a wound-related role.
Nice-to-Have Skills
Experience with Athena EMR.
Prior work in home health, autonomous practice, or wound care in a hospital or clinic setting.

Training

Workload
Daily patient volume varies based on geography and case complexity (typically 2-6 patients per day).
Charting completed remotely (iPad and laptop provided).
Patient population primarily adults (approximately 90% geriatric).
High-acuity wound cases (Stage 3-4) common.
