Advanced Practice Provider (Nurse Practitioner) – Primary Care/Nephroology

Location: Casa Grande & Maricopa, AZ (Onsite/In-Home)

Schedule: Monday–Friday | 8:00 AM–5:00 PM (1-hour lunch)

Salary: $140,000–$150,000 annually

We're seeking an experienced Nurse Practitioner (NP) to join a growing, value-based care organization dedicated to providing comprehensive primary care for patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD). This is an excellent opportunity for a provider who enjoys building long-term patient relationships while delivering high-quality, patient-centered care in both the home and virtual settings.

Position Overview

As an Advanced Practice Provider, you'll manage your own panel of medically complex patients, providing comprehensive primary care with a focus on improving outcomes, reducing hospitalizations, and enhancing quality of life. This role combines in-home visits (80%) with telehealth visits (20%), allowing providers to develop meaningful relationships with patients while working within a collaborative, multidisciplinary care model.

This position serves patients throughout the Casa Grande and Maricopa, AZ communities.

Responsibilities

• Provide comprehensive primary care to patients with chronic kidney disease (CKD), end-stage renal disease (ESRD), and multiple chronic conditions.
• Manage an assigned patient panel while developing individualized treatment plans to improve health outcomes.
• Complete approximately 5 patient visits per day (20 visits per week).
• Conduct approximately 80% of patient visits in the home and 20% via telehealth.
• Perform post-hospital transition visits within 5 days of discharge to support continuity of care and reduce avoidable readmissions.
• Maintain timely and accurate documentation while meeting note completion turnaround expectations.
• Collaborate with physicians, nurses, care coordinators, and interdisciplinary teams to provide coordinated, value-based care.
• Educate patients and caregivers on chronic disease management, treatment plans, and preventive health strategies.
• Support organizational quality initiatives focused on improving patient outcomes and reducing total cost of care.

Required:

• Licensed Nurse Practitioner eligible to practice in Arizona.
• Minimum of 3–5 years of experience practicing as a Nurse Practitioner.
• Strong background in Primary Care.
• Experience independently managing complex adult patient populations.
• Excellent clinical judgment, communication, and documentation skills.
• New graduates will not be considered.

Preferred:

• Experience working within a Value-Based Care model.
• Previous experience providing in-home medical care or home health visits.
• Experience caring for patients with chronic kidney disease, end-stage renal disease, or other medically complex populations.

• No weekends
• No alternative schedules or 4/10 workweeks available
