Position Overview:
The Telepharmacy Pharmacist plays a critical role in providing remote pharmacy services over night to hospitals and healthcare facilities across multiple states. Operating within the Medication Review Telepharmacy Division, this pharmacist ensures safe, accurate, and timely medication order entry and review. The role involves direct collaboration with healthcare providers to enhance patient outcomes and maintain the highest standards of pharmacy care in a remote setting. Medication Review provides 24/7 remote order entry services for its clients, and pharmacists must be prepared to support coverage across various shifts as needed.

Key Responsibilities:
- Review and verify physician medication orders for appropriateness, dosage, interactions, and allergies.
- Conduct clinical reviews of patient profiles, medication regimens, and lab results.
- Communicate directly with physicians and nursing staff to make therapeutic recommendations.
- Serve as a clinical resource for medication information, providing literature research and evaluations as needed.
- Review medication histories and assay reports to ensure clinical accuracy and effectiveness.
- Verify automated dispensing device (ADD) fills and maintain compliance with regulatory standards.
- Collaborate with healthcare teams to support transitions of care and optimize pharmacotherapy.
- Maintain accurate documentation and communication in pharmacy software platforms and facility records.
- Participate in quality assurance and performance improvement initiatives.
- Support other clinical pharmacy services and projects as needed.
- May supervise or provide guidance to pharmacy technicians and support staff.
Preferred:
- Experience in a telepharmacy or remote order entry setting.
- Familiarity with controlled substance verification procedures and multi-state pharmacy regulations.
Work Environment:
- Fully remote position supporting hospitals and facilities across the Northwestern United States.
- Must have access to a secure, quiet workspace with reliable high-speed internet.
- Evening, weekend, or on-call shifts may be required based on staffing needs and client coverage.

Required:
Active Pharmacist License in good standing, with licensure (or eligibility) in Washington, Oregon, Idaho, Montana, and Alaska.
Hospital pharmacy experience with a strong clinical background.
Proficiency with remote order entry systems such as Epic, CPSI, Meditech, Cerner, HMS, and/or Paragon.
Exceptional verbal and written communication skills.
High attention to detail and accuracy in a fast-paced environment.
Ability to work independently and as part of a collaborative remote team.
Flexible availability, including evenings, weekends, and holidays as needed.
