Position Overview
We are seeking a compassionate and experienced RN Case Manager to join our hospice team. In this role, you will manage a caseload of patients receiving end-of-life care, ensuring comfort, dignity, and quality of life through individualized, patient-centered care plans.
The ideal candidate is a strong clinician with hospice experience who is comfortable supporting patients and families through the end-of-life process and collaborating closely with an interdisciplinary care team.
Key Responsibilities
Case Management & Care Planning
- Manage a defined caseload of hospice patients across home and/or facility settings
- Conduct comprehensive assessments and ongoing reassessments
- Develop and implement individualized hospice care plans in collaboration with the interdisciplinary team
- Coordinate all aspects of patient care to ensure continuity and quality
Clinical Care
- Provide skilled nursing care, including pain and symptom management
- Administer medications and treatments as ordered
- Monitor patient condition and respond to changes appropriately
- Ensure care aligns with hospice philosophy and physician orders
Patient & Family Support
- Serve as primary point of contact for patients and families
- Provide education and emotional support related to disease progression and end-of-life care
- Guide families through the hospice process and care decisions
- Participate in on-call rotation as required
Interdisciplinary Collaboration
- Work closely with physicians, social workers, chaplains, and hospice aides
- Participate in interdisciplinary team (IDT) meetings
- Communicate changes in patient status and care needs
Documentation & Compliance
- Maintain accurate, timely, and compliant clinical documentation
- Ensure adherence to all Medicare hospice Conditions of Participation and regulatory requirements
- Participate in audits, surveys, and quality improvement initiatives
Qualifications
- Active Registered Nurse (RN) license in Georgia
- 1-2 years of nursing experience required; hospice or home health experience strongly preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Compassionate, patient-centered approach to care
- Ability to work independently and manage a caseload effectively
