Support Coordinator – Make a Difference Every Day

Hybrid Position | Full-Time | Monday–Friday | 8:00 AM–5:00 PM

Starting Earnings: $38,000 annually (Base Salary Performance Bonus Opportunities)

What You'll Do

As a Support Coordinator, you will serve as an advocate, resource, and guide for individuals receiving Medicaid Waiver services by:

  • Coordinating person-centered services that promote independence and quality of life.
  • Developing, implementing, and monitoring individualized service plans.
  • Conducting assessments and documenting participant progress.
  • Connecting individuals and families with community resources and service providers.
  • Coordinating with healthcare providers, social service agencies, schools, and other professionals.
  • Educating participants about available service options so they can make informed decisions.
  • Monitoring service effectiveness and recommending changes as participants' needs evolve.
  • Maintaining accurate documentation while ensuring compliance with Medicaid Waiver requirements.
  • Building trusting relationships with participants, families, and community partners.
  • Advocating for participants to ensure they receive the services and support they need.

Qualifications Required

  • Bachelor's degree in:
  • Psychology
  • Sociology
  • Social Work
  • Criminal Justice
  • Education
  • Human Services
  • Or another closely related human services field

Preferred

  • Experience in case management, care coordination, or social services
  • Experience working with individuals with developmental disabilities
  • Knowledge of Medicaid Waiver services is a plus (training provided)

Schedule

  • Full-Time (40 hours per week)
  • Monday–Friday
  • 8:00 AM–5:00 PM

Compensation

Starting earning potential of $38,000 annually, including base salary and performance bonus opportunities.
