What You'll Do
As a Support Coordinator, you will serve as an advocate, resource, and guide for individuals receiving Medicaid Waiver services by:
- Coordinating person-centered services that promote independence and quality of life.
- Developing, implementing, and monitoring individualized service plans.
- Conducting assessments and documenting participant progress.
- Connecting individuals and families with community resources and service providers.
- Coordinating with healthcare providers, social service agencies, schools, and other professionals.
- Educating participants about available service options so they can make informed decisions.
- Monitoring service effectiveness and recommending changes as participants' needs evolve.
- Maintaining accurate documentation while ensuring compliance with Medicaid Waiver requirements.
- Building trusting relationships with participants, families, and community partners.
- Advocating for participants to ensure they receive the services and support they need.
Qualifications Required
- Bachelor's degree in:
- Psychology
- Sociology
- Social Work
- Criminal Justice
- Education
- Human Services
- Or another closely related human services field
Preferred
- Experience in case management, care coordination, or social services
- Experience working with individuals with developmental disabilities
- Knowledge of Medicaid Waiver services is a plus (training provided)
