Job Title: Physical Therapist (PT)
Location: Fort Wayne, IN 46805
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Mileage Reimbursement: $0.43 per mile
Preferred Experience & Skills
- Exercise programming and rehabilitation
- Home health experience required
- Proficiency with HCHB / OASIS documentation
- Strong clinical assessment and care-planning skills
Key Responsibilities
- Perform comprehensive physical therapy evaluations and treatments per physician orders
- Review patient diagnoses, medical history, precautions, and contraindications
- Develop individualized, measurable treatment plans to maximize patient independence and function
- Reassess patient progress regularly and modify treatment plans as needed
- Communicate patient status and progress with physicians, care teams, patients, and families
- Coordinate discharge planning and consult with medical providers as appropriate
- Recommend and assist with adaptive equipment to enhance patient function
- Complete accurate evaluation, treatment, and billing documentation in compliance with company, state, and federal requirements
- Adhere to HIPAA, corporate compliance programs, and professional practice standards
- Supervise Physical Therapist Assistants and Aides in accordance with state practice acts
- Participate in staff meetings, training, and continuing education to maintain and enhance clinical skills
