Job Title: Physical Therapist (PT) – Home Health
Location: Morgantown, WV 26508
Hours: 40 hours/week | 8 hours/day
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (No weekends, no on-call)
Required & Preferred Qualifications:
- WV Physical Therapist License
- Home health experience, including OASIS documentation
- Homecare Homebase (HCHB) experience preferred
Key Responsibilities:
- Evaluate and treat patients per physician orders
- Develop and adjust individualized treatment plans
- Monitor patient progress and modify interventions as needed
- Communicate with physicians, care teams, patients, and families
- Recommend adaptive equipment to improve patient function
- Maintain documentation and supervise PTAs/Aides
- Participate in staff development and continuing education
