Position Summary
Join the GW Advanced Heart Failure Program, a dedicated team providing comprehensive care to patients with advanced heart failure, amyloidosis, sarcoidosis, hypertrophic cardiomyopathy, and related cardiovascular diseases. This ambulatory role focuses on heart failure disease management and general cardiology while integrating cutting-edge therapies, multidisciplinary care, and a commitment to safety and quality outcomes.
Key Responsibilities
- Deliver exceptional patient care, including diagnostic and therapeutic management, in an outpatient setting.
- Collaborate with attending physicians to provide autonomous care, including medical histories, physical exams, and formulation of treatment plans.
- Educate patients and families on care plans and available resources.
- Partner with inpatient teams to improve the quality of care across the healthcare system.
- Ensure compliance with documentation, billing, and regulatory requirements.
- Participate in quality improvement projects, research, and protocol development.
Program Highlights
- Offers heart failure disease management, outpatient diuretic services, and advanced therapies such as mechanical circulatory support.
- Multidisciplinary care team dedicated to improving patient outcomes in the Nation’s Capital.
- Outpatient role, Monday through Friday, with administrative time allocated.
Preferred Qualifications
- APP experience in advanced heart failure or cardiology.
- Experience in acute care settings (ED, ICU, or Step-Down) is a plus.
Minimum Qualifications
- NP: Master’s degree in nursing and ANCC certification.
- PA: Master’s degree as a physician assistant and NCCPA certification.
- Current licensure in DC (or ability to obtain).
- BLS/ACLS certification required.
Physical Requirements
- Ability to walk, bend, twist, and occasionally lift up to 20 lbs.
- Regular exposure to clinical settings requiring personal protective equipment.
