- Bachelor’s Degreefrom an ACPE-accredited School of Pharmacy
- Doctor of Pharmacy (Pharm.D.)from an ACPE-accredited School of Pharmacyis preferred
- Pharmacy Practice residency (PGY1) Critical Care Residency (PGY2)OR
- 5 years of hospital experience AND board certification in Critical Care noted as (BCCCP)
