A well-established family medicine practice is seeking a compassionate and skilled Nurse Practitioner to provide high-quality primary care to patients living in assisted living facilities. The ideal candidate will collaborate with physicians and clinical staff to evaluate, diagnose, and manage a wide range of acute and chronic conditions while promoting wellness and preventive care.
This role is ideal for a provider who values patient relationships, a collaborative environment, and continuity of care in an outpatient setting. This position is travelling to different ALF's in the treasure coast. A car will be provided.
Key Responsibilities
- Conduct comprehensive patient assessments, physical exams, and medical histories
- Diagnose and manage acute and chronic conditions for patients of all ages
- Develop and implement treatment plans, including medication management
- Provide patient education on health maintenance, disease prevention, and lifestyle modification
- Order and interpret diagnostic tests and labs
- Collaborate with supervising physicians and clinical staff to ensure coordinated care
- Document all patient encounters in the EMR in accordance with regulatory and clinical standards
- Participate in quality improvement initiatives and ensure compliance with clinical protocols
- Maintain positive and professional relationships with patients and their families
Qualifications
- Master’s or Doctorate degree from an accredited Nurse Practitioner program
- Active NP license
- Valid DEA license (or eligibility to obtain)
- Current ACLS/BLS certification
- Prior experience in family practice or primary care preferred (new grads welcome)
- Strong clinical judgment and ability to work independently
- Excellent communication and interpersonal skills
