Job Description:
As Quality Assurance Care Manager, you will oversee the new admissions, supervisory visits, and care plan renewals for Medi-Home’s patient roster. This is a terrific opportunity for a compassionate ,and health oriented Medical Assistant or Certified Nursing Assistant with advanced education and previous experience in a Clinical care setting or healthcare Facility
In this role, you will:
- Supervise and support direct care staff to ensure compliance with policies and regulations.
- Collaborate with the director and office administrator to develop care plans based on the patient’s individual needs under the PAS model.
- Conduct comprehensive non-skilled assessments with blood pressure checks ,complete supervisory visits, and document care plans and care plan renewals in line with this agency’s standards.
- Data entry , documentation , incident and event report management and follow-up.
- Function as a liaison among staff, community, and healthcare providers to ensure seamless communication.
- Provide in-field supervision and onsite training to the direct care workers.
- Participate in quality assurance, performance improvement, and re-training and development for all direct care staff.
- Collaborate with the compliance team and care case managers for scheduling follow- ups and supervisory visits.
- Support on-call operations, ensuring timely follow-up and accurate documentation.
Qualifications
- 10 years’ healthcare experience
- Active Medical Assistant License or Certificate with medication certification from an accredited School (License in PA).
- Medical Terminology
- Driver’s License, Full Coverage Auto Insurance.
- Personal Vehicle.
- Malpractice Insurance.
- Vital signs and medication management
- Hospital, nursing home or medical office experience required.
Location and Travel RequirementsThis is an on-site Full-Time position at our Philadelphia office (40 hours per week). This role requires routine travel between office locations up to four hours travel time and possible overnight stay quarterly between the service area for patient assessments, admissions, supervisory visit, and care plan renewals. As well as occasional travel with the director within the region/state for training opportunities.
Compensation
The anticipated salary range is annually $45,000-$47,000.
