- Performs exams, develops treatment plans, and manages acute/chronic conditions across all ages, utilizing in-person, telehealth, and telephonic modalities.
- Leads care team activities, conducts patient education, and coordinates referrals and follow-ups.
- Ensures compliance with regulatory standards, maintains accurate documentation, and participates in quality improvement initiatives.
- Provides leadership and mentorship to staff, fosters effective communication, and supports clinic programs.
- Handles administrative tasks such as scheduling, medication refills, and maintaining clinic records.
