Physician - Family Medicine/Internal Medicine

The Family Medicine Physician, supervised by the Lead Physician and Chief Medical Officer, provides comprehensive primary and urgent care to a diverse patient population, ensuring high-quality services aligned with protocols and policies.

Key Responsibilities Include

• Leading patient care using the Patient Centered Medical Home model, performing exams, assessments, and developing treatment plans for patients of all ages.
• Delivering care via in-person, telehealth, and phone, ensuring proper documentation and compliance with legal standards.
• Managing acute and chronic conditions, promoting preventive health, and coordinating referrals and follow-up.
• Mentoring staff, participating in quality improvement, ensuring regulatory compliance, and maintaining accurate records.
• Communicating effectively with team members, patients, and external agencies, and practicing cost-effective medicine.
