Seeking a PACE Primary Care Physician to provide comprehensive geriatric care in Lafayette, CO, Monday through Friday, 8am-5pm, with on-call duties approximately 5 days per month, including one weeknight and one weekend shift.
The role involves diagnosing, treating, and managing care for PACE participants and hospice patients, collaborating with interdisciplinary teams, and participating in care planning. The physician will respond to urgent visits both in clinics and patient homes, ensuring safety in all practices.
Qualifications include 2 years of primary care experience, an unencumbered Colorado medical license, DEA registration, and eligibility to bill Medicare/Medicaid. Experience with elderly populations is preferred. Candidates must have liability insurance, reliable transportation, and meet health screening requirements.
