Nurse Practitioner - Pulmonary - SNF / LTC Rehab center

We are seeking an experienced APRN in pulmonary care for a part-time, in-person role in skilled nursing and long-term rehabilitation settings.

• Deliver comprehensive pulmonary services, including assessments, diagnoses, and management of respiratory conditions such as COPD, asthma, and pneumonia.
• Manage noninvasive ventilation therapies, tracheostomy care, and coordinate weaning plans with the healthcare team.
• Perform patient evaluations, interpret diagnostics, and prescribe medications in compliance with regulations.
• Educate patients and staff, participate in interdisciplinary rounds, and contribute to initiatives reducing hospital readmissions.
• Require active APRN license, Master's degree, experience in long-term care, and strong clinical skills.
