Remote Social Worker – Community Health Integration (CHI)
Location: Florida (Remote)
Compensation: $22–$25/hour (based on experience)
Medcare is seeking a Remote Social Worker to support Medicare patients enrolled in our Community Health Integration (CHI) program. This role focuses on identifying patients' social needs, connecting them with community resources, and coordinating follow-up care. This position does not provide psychotherapy, counseling, or other clinical mental health services.
Responsibilities- Assess patients social needs and barriers to care.
- Identify challenges related to food, housing, transportation, financial needs, and other social determinants of health.
- Connect patients with appropriate community resources.
- Provide follow-up support and coordinate care.
- Document all activities in accordance with Medicare CHI requirements.
- Collaborate with the care team to improve patient outcomes.
- Bachelor's or Master's degree in Social Work or a related field.
- Experience in case management, care coordination, community outreach, or social services.
- Strong communication and documentation skills.
- LCSW is not required.
- Must be authorized to work in the U.S.
Work Location: Fully Remote (Florida applicants only).
