You will be responsible for providing high-quality, effective management for patients with debilitating chronic diseases. Working closely with the attending physician, you manage acute and chronic conditions, ensuring seamless and comprehensive care. You are an integral part of the interdisciplinary team, communicating and collaborating regularly with the Director of Nursing, social worker, and other key staff to coordinate and improve patient care.
