This is a full-time on-site role for a Long Term and Post-Acute Care Advanced Practice Provider (APP) located in Napoleon, OH. The APP will provide high-quality care to residents in long-term and post-acute care settings. Responsibilities include evaluating and managing patient needs, collaborating with interdisciplinary teams, and participating in the development of care plans. The APP will also contribute to training initiatives and foster an environment of excellence in patient-centered care.
- Experience and proficiency in Long-term Care and Elder Care
- Expertise in Nursing and Healthcare-related practices in post-acute settings
- Ability to provide patient education and staff training
- Commitment to delivering compassionate, team-based care
- Valid licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in Ohio
- Experience working in post-acute or long-term care settings is preferred
- Strong interpersonal and communication skills
