Location: Hood River, OR
Job Type: 13 Weeks Contract Possible Extensions
Shift: Day Shift 4x10 Hours (07:00 AM – 05:00 PM)
Expected Hours: 40 Hours per Week

Job Overview
We are seeking a knowledgeable and patient-focused Outpatient Pharmacist to join a dynamic healthcare team. The Pharmacist will be responsible for reviewing medication orders, dispensing medications, providing patient counseling, and ensuring safe and effective medication management. The ideal candidate will have strong clinical knowledge, excellent communication skills, and experience working in outpatient or acute care pharmacy settings.
Key Responsibilities
- Review, verify, and dispense prescription medications accurately and efficiently.
- Evaluate medication orders for appropriateness, interactions, allergies, and dosing accuracy.
- Counsel patients regarding medication usage, side effects, and treatment plans.
- Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care.
- Monitor medication therapies and recommend adjustments when appropriate.
- Utilize medication dispensing systems, including Pyxis, to ensure accurate medication distribution.
- Maintain compliance with state, federal, and organizational pharmacy regulations.
- Assist with hazardous drug handling and chemotherapy medication processes when required.
- Document pharmacy activities accurately using Epic and other pharmacy management systems.
- Support outpatient infusion services, inpatient units, emergency department, labor and delivery, and medical-surgical areas as needed.
- Participate in quality improvement initiatives and medication safety programs.

Required Qualifications
- Minimum of 1 year of experience as a Pharmacist.
- Active Oregon Pharmacist License.
- Experience in outpatient pharmacy, acute care, or infusion pharmacy settings preferred.
- Experience with Epic charting system preferred.
- Experience using Pyxis medication dispensing systems preferred.
- Chemotherapy/Hazardous Drug handling experience preferred.
- Strong clinical judgment, communication, and patient counseling skills.
