Med Staff are hiring a compassionate RN Case Manager to join our care team. This role is ideal for a nurse who values autonomy, meaningful relationships with patients and families, and the support of a collaborative team.
As an RN Case Manager, you will play a vital role in providing skilled nursing care to patients in their homes, ensuring high-quality, personalized care that promotes patient independence and wellness.
Responsibilities:
- Provide skilled nursing visits and develop individualized care plans
- Manage skilled care based on each patient’s needs and doctor’s orders
- Coordinate with interdisciplinary team members including physicians and therapists
- Serve as the main point of contact for patients and families
- Maintain accurate clinical documentation in accordance with state, federal, and organizational guidelines

Qualifications:
- Current RN license in the state of Missouri
- Minimum of one year of home health experience preferred
- Excellent clinical assessment, communication, and critical thinking skills
- Reliable transportation and valid driver’s license
