LVN Field Nurse

Job Summary
We are seeking a dedicated and compassionate Field Nurse to provide high-quality nursing care to patients in their homes. The ideal candidate will possess a strong background in various nursing skills and be committed to delivering exceptional patient-centered care. This role requires flexibility, excellent communication skills, and the ability to work independently while ensuring patient safety and comfort.

Duties

• Conduct thorough diagnostic evaluations to assess patients' health needs.
• Provide home care services, including wound care, medication management, and health education.
• Perform IV insertion and administer injections as required.
• Utilize aseptic techniques during procedures to prevent infections.
• Monitor patients with specific needs such as cardiac catheterization or neonatal ventilator support.
• Offer specialized care for patients with dementia or those living with disabilities.
• Document patient progress and maintain accurate health records using Athenahealth or similar systems.
• Collaborate with healthcare teams to develop and implement individualized care plans.

Requirements

• Valid nursing license in the state of practice.
• Proven experience in home care settings is preferred.
• Proficiency in diagnostic evaluation techniques and patient assessment.
• Experience with IV insertion and administering injections is essential.
• Familiarity with cardiac catheterization procedures is a plus.
• Knowledge of aseptic techniques for safe practice.
• Experience working with neonates on ventilators is highly desirable.
• Understanding of dementia care practices and working with individuals with disabilities.
• Strong communication skills, both verbal and written, to effectively interact with patients, families, and healthcare providers.
