MUST BE ABLE TO TRAVEL TO COVER THE FOLLOWING LOCATIONS:
Sulphur, Davis, Wayne, Ardmore areas, the counties include Love, Murray, Garvin, Carter and McClain
Qualifications
Education: Bachelor’s degree and/or one paid year of professional experience with aging or disabled population, performing duties which encompass the core functions of case management including; assessment, planning, implementation, monitoring and reassessment.

Qualifications

- Advantage Case Management Certification preferred
- Licenses/Certifications: RN/LPN
- Experience: RN/LPN with one year paid professional experience preferred.
- identify member needs, goals and abilities
- Ensure ongoing coordination.
- Prepare and submit a comprehensive, written service plan for each member
- Implement certified ADvantage service plan
- Monitor delivery of services
- Identify high risk situations
- Orient and educate members
- Modify services to meet needs of members.
- Ensure confidentiality according to all relevant laws and regulations.
- Maintain adequate records
- Justifies expenditures and monitors and contains cost of plan.
- Assist in implantation of inservice education for field employees
- All other duties and responsibilities assigned by management.
- Excellent verbal, interpersonal and written communication skills;
- Excellent problemsolving skills;
- Ability to use a computer and relevant data bases, especially for required reporting;
- Ability to organize and manage a caseload in accordance within agency standards and in compliance with policies and procedures of contracted programs
