What are the responsibilities and job description for the Patient Navigator, Lung Cancer Screening position at Med Center Health?
Serves as a lung cancer Navigator and patient advocate with the mission to improve the care experience for patients undergoing lung cancer screenings. Facilitates the multidisciplinary clinical care of patients identified as participants on the Lung Cancer Clinical Care Coordination Program. Assists in the care coordination process through software applications specific to lung cancer screening. Improves patient preparedness for treatment through education and psychosocial support while also facilitating interaction between patients, their physician(s) and referrals as appropriate. Collaborates with radiology to alert the clinical care team and Multidisciplinary Cancer Conference of suspicious pulmonary lesions identified radiographically from either the lung cancer screening program, incidental nodules referred to by clinical providers, or through radiology software alerts of incidental nodules. Provides support to the clinical and leadership teams involved in program development, expansion, and successful achievement of programmatic goals.
Minimum Qualifications
- Two years of experience in a health-related field required.
- Bachelor’s degree in a health-related field or equivalent experience required.
