Hospice/Medical Social Worker

Pay: $55/hr ($2,200/week)

Join the Evergreen Hospice team at Samaritan Health Services and provide compassionate psychosocial support to patients and families during the final stages of illness. This role offers the opportunity to make a direct difference through patient-centered care delivered in homes, skilled nursing facilities, assisted living facilities, and adult foster homes.
Key Responsibilities
- Provide medical social work, counseling, and emotional support to hospice patients and families.
- Assess psychosocial needs and develop appropriate support plans.
- Collaborate with nurses, providers, spiritual care, hospice aides, volunteers, and bereavement teams.
- Support patients and families with end-of-life care needs and available community resources.
- Serve patients across Linn, Benton, Marion, Polk, Lincoln, and Tillamook Counties.
Requirements
- Oregon LMSW or CSWA license required at submission.
- Healthcare Provider BLS required.
- Valid driver's license and reliable personal vehicle with required auto insurance.
- Oregon LCSW preferred.
- Hospital or medical social work experience preferred.
