Registered Nurse (RN) – Hospice Admissions
**Affinity Hospice is seeking a compassionate and experienced **Registered Nurse (RN) to join our team serving League City and surrounding areas.
Position Summary
The Registered Nurse will provide direct and indirect patient care in accordance with Advanced Hospice Care’s policies, procedures, and standards of practice. The primary responsibility of this position is to conduct field visits for hospice admissions, ensuring patients and families receive a thorough, compassionate, and professional start to their hospice journey.
This position is primarily focused on admissions and field visits rather than serving as the patient’s primary caregiver. Depending on patient census and operational needs, the RN may also be asked to make additional patient visits.
Key Responsibilities
- Conduct comprehensive hospice admission visits in the patient’s home or appropriate care setting.
- Complete nursing assessments and required admission documentation accurately and timely.
- Coordinate with physicians, hospice team members, patients, and families to support continuity of care.
- Provide patient and family education regarding hospice services and the plan of care.
- Deliver compassionate, patient-centered nursing care consistent with hospice standards and organizational policies.
- Make additional patient visits as needed based on patient census and agency needs.
- Maintain accurate and timely clinical documentation.
- Adhere to all applicable regulatory, professional, and organizational requirements.
Qualifications
- Current Registered Nurse (RN) license in the applicable state.
- Degree in Nursing from an accredited nursing program.
- Minimum of one year of hospice nursing experience required.
- Strong clinical assessment, communication, and organizational skills.
- Ability to work independently in a field-based environment.
- Reliable transportation and willingness to travel throughout League City and surrounding areas.
