MECA Therapies is seeking a compassionate and motivated Delegated Care Coordinator to join our growing Maternal Health Program.

Position Summary

The Delegated Care Coordinator provides comprehensive care coordination services to pregnant and postpartum members who may be experiencing medical, behavioral, or social risk factors. Working closely with healthcare providers, community partners, and families, this position ensures members receive timely prenatal care, postpartum support, education, and access to essential community resources.

Services are delivered through a combination of home visits, virtual visits, telephone outreach, and community-based care coordination.


Qualifications

Required

  • Bachelor's degree in Social Work, Public Health, Human Services, Nursing, Psychology, Sociology, Early Childhood Education, or a related field.

OR

  • Minimum of two (2) years of experience in case management, care coordination, social services, healthcare, behavioral health, community health, or a related field.
  • Valid driver's license, reliable transportation, and automobile insurance.
  • Ability to travel throughout assigned service areas.
  • Strong organizational, communication, and documentation skills.
  • Ability to maintain confidentiality and comply with HIPAA requirements.

Preferred

  • Experience in maternal health, prenatal care, postpartum care, or women's health.
  • Knowledge of Medicaid managed care or care coordination programs.
  • Community Health Worker (CHW) experience.
  • Experience serving high-risk or underserved populations.
  • Bilingual (English/Spanish) strongly preferred.

Essential Responsibilities

  • Conduct comprehensive intake assessments to identify medical, behavioral, social, and environmental needs.
  • Develop individualized care plans that support healthy pregnancies and positive birth outcomes.
  • Coordinate care with physicians, hospitals, specialists, behavioral health providers, and community agencies.
  • Conduct home visits, virtual visits, and telephone follow-ups to assess member needs and monitor progress.
  • Educate members on prenatal care, postpartum recovery, infant care, breastfeeding, safe sleep, nutrition, and maternal wellness.
  • Connect families with community resources, including transportation, housing assistance, food programs, childcare resources, behavioral health services, and financial assistance programs.
  • Advocate for members to reduce barriers to healthcare and improve access to needed services.
  • Monitor member progress and adjust care plans as needed throughout pregnancy and the postpartum period.
  • Maintain accurate documentation within electronic health record systems and care management platforms.
  • Collaborate with interdisciplinary teams to promote coordinated, culturally responsive care.
  • Participate in quality improvement initiatives and community outreach activities.
  • Maintain compliance with all state, federal, Medicaid, HIPAA, and organizational requirements.

Caseload & Productivity Expectations

  • Manage an assigned caseload of approximately 60 members, depending on program needs and complexity.
  • Conduct regular outreach and follow-up to ensure members remain engaged in care.
  • Complete required home visits and telephonic contacts according to program standards.
  • Meet established productivity and documentation expectations while maintaining high-quality member-centered services.

Documentation & Compliance

  • Complete timely and accurate documentation of assessments, care plans, member contacts, referrals, and outcomes.
  • Maintain confidential electronic records in accordance with HIPAA requirements.
  • Track member progress toward established goals.
  • Submit documentation within required timelines.
  • Participate in quality assurance reviews and documentation audits.
  • Maintain compliance with all payer, regulatory, and organizational documentation standards.

Professional Development

Delegated Care Coordinators are expected to:

  • Attend required orientation, trainings, and continuing education.
  • Participate in ongoing professional development related to maternal health, care coordination, trauma-informed care, and cultural competency.
  • Maintain knowledge of community resources and best practices in maternal and infant health.
  • Participate in team meetings, case conferences, and interdisciplinary collaboration.
