Position Summary: The Case Manager will provide comprehensive case management services to individuals we serve either in our supportive transitional housing and/or our Resource Center. This role involves assessing client needs, developing individualized service plans, and connecting clients with community resources to support their journey toward self-sufficiency.Key Responsibilities:● Conduct intake assessments to determine client needs and eligibility for services.● Develop, implement, and monitor individualized service plans for each client.● Provide ongoing case management, including regular follow-up and reassessment.● Connect clients with community resources, such as healthcare, employment, education, and housing services.● Advocate on behalf of clients to ensure they receive appropriate services and support.● Maintain accurate and up-to-date case files and documentation.● Collaborate with other staff and community partners to coordinate services and support.● Participate in team meetings, training, and professional development activities.● Perform other duties as assigned by the Program Manager.Qualifications:● Minimum of 2 years of experience in case management or a related field.● Knowledge of homelessness, substance abuse, mental health, and community resources.● Strong organizational and time management skills.● Excellent communication and interpersonal skills.● Ability to work independently and as part of a team.● Proficiency in Microsoft Office and case management software.● Valid driver’s license and reliable transportation.Our organization hosts occasional events that are optional to attend but if attended regular work hour pay will be compensated for time worked during said event.Case Manager Skills and Qualifications:Documentation skillsDecision makingResearch skillsVerbal & Verbal communicationPeople skillsResolving conflictComputer SavyyEducation, Experience, and Licensing Requirements:High School Diploma or GEDENTRY-LEVEL okay2 years customer service experience, preferably in human services, nonprofit, possess demonstrated commitment to client outcomes.Professional or personal experience supporting individuals with homelessness, mental health, substance use disorderMust have a valid in-state driver’s license or state issued identification.Must be over 21 years old.Call to Action:If you are a student of social work or case management this is a perfect opportunity for you to get hands on experience working with vulnerable populations and make a direct impact.Company DescriptionMeasure of Hearts Foundation is a 501(c)(3) nonprofit organization serving vulnerable populations in South Los Angeles through our community-based Resource Center.
