Meadowbrook Post Acute is seeking a detail-oriented and organized Medical Records Designee to join our growing team. We are a skilled nursing facility dedicated to providing compassionate care and quality service to our residents and families. The ideal candidate will be dependable, professional, and knowledgeable in maintaining accurate and confidential medical records in accordance with state and federal regulations.
Position Summary
The Medical Records Designee is responsible for maintaining resident medical records, ensuring documentation compliance, assisting with audits, processing admissions and discharges, and supporting the interdisciplinary team with record management and regulatory requirements.
Essential Duties and Responsibilities
- Maintain resident medical records in accordance with facility policies and state/federal regulations
- Assemble, thin, audit, and maintain resident charts
- Ensure physician orders, progress notes, assessments, and ancillary documentation are properly filed and completed
- Monitor records for completeness and compliance
- Prepare charts for admissions, transfers, and discharges
- Manage electronic and paper medical record systems
- Process requests for medical records while maintaining HIPAA compliance
- Assist with surveys, audits, and regulatory requests
- Coordinate with nursing, therapy, social services, and administration regarding missing or incomplete documentation
- Track and follow up on physician signatures and incomplete records
- Maintain confidentiality of resident information at all times
- Assist with scanning, uploading, and archiving documents as needed
Qualifications
- Previous experience in medical records preferred, especially in a skilled nursing facility or healthcare setting
- Knowledge of medical terminology and healthcare documentation practices
- Familiarity with HIPAA regulations and confidentiality standards
- Experience with electronic medical record systems preferred
- Strong organizational and time management skills
- Ability to work independently and prioritize multiple tasks
- Excellent communication and interpersonal skills
- High school diploma or equivalent required
Preferred Experience
- Skilled nursing facility experience
- PCC or other EMR experience
- Understanding of Medicare, Medi-Cal, and managed care documentation requirements
