Social Services Assistant
Meadowbrook is looking for a compassionate and dedicated Social Services Assistant to work hand in hand with our Social Services Director in supporting the emotional, social, and overall well-being of our residents.
Skilled nursing facility experience is preferred, but we are looking for someone who is compassionate, organized, and passionate about advocating for residents and their families.

Responsibilities
- Assist the Social Services Director with day-to-day social services needs
- Support residents and families with social, emotional, and psychosocial needs
- Assist with assessments, care planning, and required documentation
- Advocate for resident rights, dignity, and quality of life
- Provide support to residents and families during transitions and adjustments to long-term care
- Assist with discharge planning and connecting residents with community resources
- Participate in interdisciplinary team meetings and care planning
- Help address resident concerns and promote a positive resident experience
- Maintain compliance with facility policies and state and federal regulations
Skilled nursing facility experience preferred.
